Unable to Remove Conduit from Windows 8

I am trying to remove conduit from Windows 8. I have looked for it on the Control Panel under Conduit, Conduit/toggle, Utorrent, Yontoo and Babylon and could not find any of those or any recently installed suspicious programs.

I then went to the Explorer homepage, Tools, Manage add-ons and found this suspicious add-on: Perk Prize Panel and under publisher it said Control Name is not available and it is enabled. I tried to disable it and the system would not let me. I then selected more information and found the following:

Version; Not available

File date: (this is blank)

Date last Accessed: Not Available

I would appreciate any help that can be provided to remove this
