not sure.

if you google them they seem to be
related to a service area of some sort.

one of them is a toll free number,
so you might want to call it and
see what you get.

the fields may be part of a
crashed database or poorly
designed software that uses
the fields or maybe even
a phishing expedition whereas
the fields were part of an
html form....

can't really tell you except
keep your guard up and keep
you protection up as well.....

My guess is that you are using an HP computer and these are the
pre-configured settings for remote access to your computer in the event that
you need assistance from HP.
Google this for more info;
