TweakUI does not remember X-mouse setting

I was using X-mouse from tweakui previously but recently disabled it
in favor of the "True X-Mouse Gizmo for Windows" <http://
fy.chalmers.se/~appro/nt/TXMouse/>.

I find, however, that on reboot the X-mouse setting in tweakui is
always reenabled and I have to manually disable it. The OS doesn't
seem to be remembering the setting across sessions.

Any ideas why this might be happening? Isn't tweakui just setting a
registry key?

Yes, it does, and clearly you have a conflict here.

Try this .reg tweak file:
http://www.kellys-korner-xp.com/regs_edits/savesettings.reg

Also check out the permissions for the registry key:
HKEY_CURRENT_USER\Control Panel\Mouse

Make sure that the account you are using has full control to this key.
