There is no way to do such a survey because:

1) An anonymous survey method, would be abused.
2) A survey where the participants are identified,
will not be participated in. Nobody gives up a
good working email address, for some kind of "prize".

Best method - be a passive observer. You'll learn everything
you need to know. And some things you didn't want to know...
