Running Vista Ultimate 32-bit SP1. I get the Security Update for Microsoft
XML Core Services 4.0 Service "successfully" updated each day. Every day
Windows Update says I have one "important" update to install and it is
always KB936181-the Security Update for Microsoft XML Core Services 4.0
Service.
Security Update for Microsoft XML Core Services 4.0 Service Pack 2
(KB936181) may repeatedly appear in the update list on Microsoft Update
or on Windows Update http://support.microsoft.com/kb/941729
Try the steps listed in the KB article, Jerry. Some 'thing' is
preventing the msxml4.dll from being swapped out because it's locking it.

Thanks a lot. It turned out that executing this line solved it:
C:\windows\system32\regsvr32.exe msxml4.dll
Credits go to Don in Tucson who pointed this out.

I am having the same problem however, when I type in the information at the
prompt, it tells me access denied. I am the admin and the only user of this
laptop. How do I get this to rewrite the file as old and reinstall if I am
being denied access?

This is Vista, correct ? You'll need to run an Elevated Command Prompt.
Locate cmd.exe and *right* click it, choose 'Run as administrator',
follow the prompts.
BTW, the command to register the file is
regsvr32 msxml4.dll
IF that does not resolve the reoffering issue, then please see:
