How to add a new file extension in vista

XP has the ability to add a new file extension
in control panel/file options/file types/new(button).
I have not found this button or its equivalent in vista
How do I add a new file extension in vista?
In vista, control panel/default programs/set associations seems to be
a misnomer since it does not give the option of adding a non-existent
file extension association.

There's no need to manually register a new file extension.
Unrecognized file extensions will still show 'Open' on the right-click
context menu. Simply find a file with the extension that you wish to
add, right-click it, select 'Open' and follow the prompts to associate
the file with an application or command.

To change a file extension association go to Control Panel->Programs-
>Default Programs
 
Create a file using Notepad, and save it with the extension you want to add. Double-click the file and in the Open With dialog, choose the application from the list. Place a checkmark near "Always open...." and click OK.

This adds the new file type for the *current* user account.
